Perforations are pathologic or iatrogenic communications between the root canal system and the attachment apparatus. The clinician must be particularly concerned about avoiding the tooth's perforations during endodontic therapy since a perforation will necessitate additional treatment. The perforation creates an "additional" portal of exit in the root canal system. Once identified, it must be sealed as quickly as possible since periodontal involvement arising from the perforation can become irreversible with time. If a perforation occurs, the tooth does not necessarily require surgery, intentional replantation, or extraction; in fact, it can be treated successfully in a conservative manner and continue to function like the perforation was never done. Today, there is no reason to believe that the tooth will be lost prematurely because of this complication.

Prof Arnaldo Castellucci

From 1978 to 1980 he attended the Continuing Education in Endodontics courses at the Boston University School of Graduate Dentistry with Prof. Herbert Schilder. Since 1979 he has limited his professional career to Endodontics.

He is a Past President of the International Federation of Endodontic Associations (IFEA) and of the Italian Society of Endodontics (SIE – Società Italiana di Endodonzia), he is an Active Member of the American Association of Endodontists (AAE) and of the European Society of Endodontology (ESE). An internationally acclaimed relator, he has authored the textbook EndodonziA, published by Casa Editrice Martina, and the new edition, EndodonticS, published by Il Tridente in english and distributed by Editore Martina of Bologna.

From 1983 to 2000 he was a Visiting Professor of Endodontics at the Graduate Dental Course of the Università di Siena and from 2000 to 2011 he was a Visiting Professor of Endodontics at the Graduate Dental Course of the Università di Firenze. Since 2013 he has been a Professor at the School of Specialization in Oral Surgery at the Università Federico II of Naples.

Since 2015 he has been a Visiting Professor at the Graduate Dental Course of the Università di Cagliari.

He has been Managing Director of the Italian Journal of Endodontics (Giornale Italiano di Endodonzia) and Managing and Scientific Director of the Endodontic Informer magazine (L’Informatore Endodontico), he is Editor in Chief of the Endo Tribune magazine, President of the Warm Gutta-Percha Study Club and he is the founder of the Centre for Teaching Micro-Endodontics (Centro per L’Insegnamento della Micro-Endodonzia), situated in Florence, where he teaches and carries out theoretical and practical courses concerning Clinical and Surgical Endodontics with a microscope.
